Our patient forms can be downloaded and completed to make your first visit about getting to know you. Your first visit with us will include a thorough history, examination, and in most cases spinal x-rays. Dr. Alex and Dr. Megan will guide you through the process and teach you everything you need to know about your spine, your chiropractic care, and how to improve your overall health.
